5th Grade Yearbook Dedication Payment Form Only

Deadline is January 31, 2025 to submit payment.

Deadline is March 14, 2025 to submit dedication artwork.

1/4 Page Appx. 3.5 WIDE x 4.75 in HEIGHT

1/2 Page Appx. 7in WIDE x 4.75 in HEIGHT

 Full Page Appx. 7in WIDE x 9.5 in HEIGHT

All Dedications Must Be Submitted Using this Instructions:

  1. Using MS Word or Adobe or similar graphic design programs like Canva to create dedication using the page size dimensions. 
  2. Photos and wording must fit within the dimensions. Please use HIGH RESOLUTION PHOTOS.  Many photos from social media will be in low resolution and may be blurry.
  3. Send design in either .DOC, .PDF or. JPG file formats to AUSTINES.PTO@gmail.com with your student's name in the subject line.
  4. MAKE SURE to SPELL CHECK and use high resolution photos.
  5. Once it's ready, Email Austines.pto@gmail.com.  A volunteer will confirm receipt of your submission and payment. 
  6. NEED DESIGN HELP? - Select DESIGN HELP on the order form for $5. A volunteer will design your dedication and submit for your approval.

Parent is responsible for creating and submitting dedication to the Austin PTO Yearbook. The Austin PTO is not responsible for spelling errors or design issues if designed by a parent. Austin PTO will confirm if your dedication has been submitted.
